Prescription, OTC & Supplement History

What are you taking—including everything outside your prescriptions?

What’s included

We bring prescriptions, over-the-counter medicines, vitamins, herbs and supplements into one source-linked history. We record product and ingredient names, strength, amount, route, frequency, reason for use, prescriber or recommender, start and stop dates, changes and recorded reactions where supplied. Receipts and order histories establish purchases; patient accounts and clinical records establish what each source says about use.

What comes back to you

THE WHOLE MEDICATION PICTURE

What’s in the cupboard?
What’s missing from the chart?

A daily vitamin. An occasional allergy tablet. An herbal blend ordered online. Bring the information you have; we help identify the details still needed.

Records beyond the prescription list

Clinical lists and pharmacy histories, product-label photographs, store receipts, emailed order confirmations, retailer order exports and your account of what you take can all contribute.

For online purchases, supply an export or receipt. Any direct access must be separately arranged and authorized; this website does not connect to shopping or pharmacy accounts.

Questions we carry for you

Which product and formulation? What ingredients and strength per serving? How much do you take, how often and why? When did you start, change or stop? Who prescribed or suggested it? Was it bought for you, and do you still use it?

We record missing dates, unclear labels, duplicated ingredients and differences between sources as questions for review.

A purchase and a history of use

Each entry identifies whether it comes from a clinical record, a dated patient account or purchase evidence only. A receipt date remains a purchase date; it does not become a start date.

Your appointment list separates reported current use from entries still awaiting confirmation, with a source index behind the detail.

FOR CLINICIANS & PRACTICES

The patient is new.
The history isn’t.

Scattered records can make the first clinical review a search across providers and years. Brightly connects the documented history into a source-cited account for the clinician developing the treatment plan.

For clinicians & practices

New-Patient Record Examination

A new patient. Several providers. Years of records to connect before planning care.

What’s included

We organize and examine the supplied records across providers: diagnoses, prescriptions, reactions, procedures, tests, encounters and care transitions. Each entry identifies its source, including where the record contains a patient-reported history. Where supplied, we include OTC medicines and supplements, identify purchase records separately from reported use, and preserve discrepancies for review.

What comes back to you

A source-cited intake history, medication chronology, provider index, conflicting entries and missing records for the clinician’s review.
