The recorded care, ready for the next clinician.
Who received care stays consistent throughout the packet. Who provided it, what was recorded, where and when care occurred, and the documented why and how are organized from the available sources.
The wording matters.
Diagnoses, medicines, procedures, tests, encounters and recorded findings retain their source wording. Any recorded standard code is noted alongside the relevant entry, with its code system where identified.
Each finding points back to the document and location that support it. Differences, missing dates and unreadable passages remain visible. A code or diagnosis is not supplied merely because it seems likely.
