The scope
We bring prescriptions, over-the-counter medicines, vitamins, herbs and supplements into one source-linked history. We record product and ingredient names, strength, amount, route, frequency, reason for use, prescriber or recommender, start and stop dates, changes and recorded reactions where supplied. Receipts and order histories establish purchases; patient accounts and clinical records establish what each source says about use.
What you receive
An appointment list and detailed timeline showing clinically recorded medicines, patient-reported use and purchase-only entries separately, with source dates, differences and questions to confirm.
Where we begin
Which prescriptions, OTC medicines, vitamins, herbs and supplements should we include—including occasional use?
What strength, amount and frequency do you actually take, and when did you start or stop?
Which clinical lists, pharmacy records, product-label photos, receipts or online order exports are available?
Are any purchases for someone else, unused, returned or a different formulation?
Which dose changes, reactions or differences between lists need tracing?
